1. All cardholders will be subject to identity verification and approval in accordance with applicable law and bank policy. Obtaining Your Card: The USA PATRIOT Act is a federal law that requires all financial institutions to obtain, verify, and record information that identifies each person who opens a Card Account. What this means for you: When you open a Card Account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
This card is issued by BankFirst, Sioux Falls, SD; Member FDIC. Your use of the card will be subject to the terms and conditions of a Cardholder Agreement. In addition to the one-time non-refundable Enrollment Fee of $9.95, cardholders will be subject to monthly maintenance fees and other charges as set forth in the Cardholder Agreement. For security reasons, this card will contain minimum and maximum load restrictions which are subject to change.

2. Cash Withdrawal Fee of $2.00, plus applicable ATM operator fees, apply.

3. This card subject to load and account balance limits. If a direct deposit transaction from your employer would exceed these limits, the transaction will be denied and you will need to obtain your pay from your employer some other way. Only one employer may send direct deposits to your card. See Cardholder Agreement for details.
Direct Deposit feature is subject to your employer's support of direct deposit for payroll: Check with your employer to find out how quickly direct deposit will commence. Funds availability is subject to timing of employer's funding

4. The $4.95 Monthly Account Maintenance Fee will be waived for the statement period immediately following the date on which a direct deposit load is posted to your card account.
